Anti-Coagulation is one of many disease management programs that we offer. While teaching patients and caregivers about their disease and the process of living with their disease (how to take their medications, monitor side effects, protect themselves from injury, foods to avoid, etc.), we offer the service point of care Protime/Inr results. This allows us to determine the effects of their "blood thinner" and report the results to the physician while we are still in the home with the patient. By doing this, we can get physician orders and make medication changes while still in the patients home. This improves quality of care by making sure the correct dosages are known by the patient and caregiver. We currently use the CoaguChek System by Roche Diagnostics.
