Medicare-certified home health agencies are required to do a comprehensive assessment for all patients who are receiving skilled care that is reimbursed by Medicare or Medicaid, known as OASIS (Outcome and Assessment Information Set). The comprehensive assessment is conducted by a skilled health care professional, and includes evaluation of the patient's functional limitations, diagnoses, medical problems, living situation, and prognosis. OASIS data are submitted by home health agencies to the states, and subsequently transmitted to the Centers for Medicare & Medicaid Services. Health Care Pharmacy is an accredited home equipment provider with The Compliance Team, Inc. and demonstrate outstanding standards for patient safety, honesty and caring. These standards include ensuring our patients not only receive equipment in excellent condition; but also receive instruction in the use, maintenance, safety issues with home and equipment. follow-up calls and visits as warranted, accurate billing, compliance with all federal and state guidelines, and many other issues. Health Care Pharmacy has always prided themselves on their excellence in care and service to their patients and will continue to improve and develop strong principles and guidelines to ensure the best possible care available.
