Medicare-certified home health agencies are required to do a comprehensive assessment for all patients who are receiving skilled care that is reimbursed by Medicare or Medicaid, known as OASIS (Outcome and Assessment Information Set).

The comprehensive assessment is conducted by a skilled health care professional, and includes evaluation of the patient's functional limitations, diagnoses, medical problems, living situation, and prognosis. OASIS data are submitted by home health agencies to the states, and subsequently transmitted to the Centers for Medicare & Medicaid Services.
