RN Case Manager

We are looking for professional RN’s with strong clinical skills and exceptional organizational skills who demonstrate the ability to utilize critical thinking in the process of problem solving, to case manage the care of a variety of clients in the home setting. Home Care or Case Manager experience a plus. Positions require some daily travel, varied hours, and flexibility. Must be able to work independently and with direction.

Home Health Aide

Seeking an individual to provide hands on personal cares to include bathing, oral care, skin care, and hair care to in home care clients. This positions also requires the home health aide to assist with feeding, ambulation, toileting, transferring, routine exercises, and medication reminders. Light meals, and light housekeeping along with some laundry task may also need to be done. Position part time with possible working into full time.

  Home Health Aide

Seeking an individual to provide hands on personal cares to include bathing, oral care, skin care, and hair care to in home care clients.   Registered Nurses

Duties include performing initial evaluation visits to assess client's overall condition, administer services requiring substantial and specialized nursing skills such as, IV therapy, medications, treatments, dressings, and other nursing services. Qualification includes a RN license.

We currently have openings for visits in the Ames, Boone, Nevada and Des Moines locations.

  Case Managers

Duties include client assessment for Homecare appropriateness, development of comprehensive plan of cares, ability to facilitate reimbursement with third party payors, case conferencing and supervision of our field staff. Qualification includes a RN license and Home Care experience.

  Clinical Supervisor

Duties include maintaining all clinical records, accepting and processing client referrals, assessing the training needs of field staff, and monitoring case managers and field staff to ensure compliance with regulatory standards. This management-level person also will work with the Branch Administrator and serve as clinical leader for our home care staff. Previous home care experience, including familiarity with the OASIS assessment tool, and a current Iowa nursing license are required.
